If you believe you were treated unfairly or need to raise a concern about access, communication, or a civil rights issue, you can contact our office and we will work with you to review the concern.
File a Complaint With HHS OCR
You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, electronically through the OCR Complaint Portal or by mail or phone.
